Therapy to Home Program

Fair Oaks Health Care Center's Therapy to Home Program is the stepping stone between a hospital stay and returning home. Our typical residents needing sub acute care are recovering from post hospital or surgical care with physician orders for medical specialties such as physical, occupational and speech therapies, IV therapy and wound care. Our residents often face extended periods of time to recover prior to returning home, therefore our sub acute care program focuses on the total health care environment specific to the diagnosis. By offering daily activities and exercise as well as a social dining experience, our residents will experience a comfortable homelike setting while recovering.

Restorative Program

To maintain optimal physical, mental, and psychosocial functioning, Fair Oaks Health Care Center uses a whole team approach to restorative programming for our long term care residents. The support from the interdisciplinary team plays an active role in the resident maintaining their quality of life. Residents who would benefit from restorative nursing include but are not limited to inactive residents or residents experiencing natural changes from the aging process.

Outpatient Therapy Program

The Oaks Outpatient Therapy Center is designed to continue therapy after the resident has completed the Therapy to Home Program or for those residents who are living at home, but would benefit from physical, occupational or speech therapy. The Oaks Outpatient Therapy Center specializes in:

• Therapy to Home private suites include private bathroom, flat screen television with cable service, electric beds, resident controlled thermostat, generous closet space and telephone service available
• Semi Private Suites come furnished with a bed, nightstand, and closet space. We encourage residents to bring their favorite chair, quilt, and special memories to make the room feel just like home.
